Patient Centered Medical Home

As of 2011, Asheville Pediatric Associates, APA, has enthusiastically embraced the challenge to obtain Patient Centered Medical Home Certification though the National Committee for Quality Assurance, the new standard for primary care.  This is an exciting endeavor and will further enhance the foundation of trust APA has built with Western North Carolina families
What is a Patient Centered Medical Home?  It is a model of care that strengthens the pediatrician-child-family relationship by replacing episodic care with coordinated, proactive, team-based care that will benefit your child over the long-term.  Each child and their family has a relationship with one of our pediatricians who leads a team that takes collective responsibility for the child’s health care needs and arranges for appropriate care with other qualified clinicians and services. 
APA’s Medical Home will achieve these goals by exceeding the constantly evolving standards of accessibility, communication, evidence based medicine, and information technology.  Please ask your pediatrician about this new program.
